| You are submitting this report to the Association of Flight Attendants-CWA, AFL-CIO (AFA). At each AFA council office, one or more selected AFA council members who register with the AFA international office in Washington, DC will have access to
the reports filed by the AFA members at that airline or council. Summary information from reports submitted by persons other than AFA members, specifically excluding any information that could identify the person(s) reporting the incident, will also be shared with selected, registered AFA
council members affiliated only with the airline on which the incident occurred.
The AFA international office will maintain the overall database of reports. Any requests from individual council offices for incidents reported by AFA members employed at airlines other than their own will first have to be approved by the international office. AFA considers this information confidential and will not release any information that could identify the person who submitted the report to third parties without the express permission of the reporter. |
